Overview

The Primary Care Partnership strategy aims to create a genuine primary care service system where service providers work together across municipalities. This is done by assisting healthcare providers and professionals such as GP’s, Community Health Nurses, Counsellors, Allied Health Services and others to coordinate the work they have with common clients.

The service providers collectively form the primary care partnership and the South West PCP currently has 23 member agencies.

The South West PCP full works includes the three local government areas of Moyne, Corangamite and Warrnambool, is one of 32 alliances across Victoria.

The aims of South West PCP are to:

• Improve the experience and outcomes for people who use primary care services, regardless of location
• Improve the health and wellbeing of all members of our community

To achieve the aims above we are:

• Coordinating services better through implementing a common referral and information collection system
• Facilitating agencies to work together to achieve goals that benefit consumers
• Ensuring our services are responding to needs of the local community through a Consumer Reference Group which consists of local people
• Giving better information to consumers through “Information Hubs”. This has been developed by community members for the community, to assist people getting health and general service information in a convenient way
• Working with agencies to create and develop effective health promotion strategies that are relevant to South West Victoria
• To decrease common ‘above average’ medical conditions for our region by increasing awareness, healthy living plus treatment options
• Provide long term planning and strategies to continually improve the health experience of Moyne, Corangamite and Warrnambool residents
