Home and Community Care (HACC) program

Portland District Health offers a range of health services for people who meet the criteria for HACC:

• Are a carer for a person with a disability

• Aged 65 years and over

• Podiatry

• Dietetics

• Occupational Therapy

• District Nursing

• Community Transport

The Home and Community Care (HACC) program supports frail aged people, younger people with disabilities, and carers. It is designed to support people whose capacity for independent living is at risk, or who are at risk or premature or inappropriate admission to long-term residential care.

Eligibility does not depend on age or income. You do not need a referral to see a clinician.

This organisation receives Commonwealth and Victorian Government funding support under the HACC program.
