ADULT DAY ACTIVITY PROGRAMS
A service which focuses on maintaining an individuals ability to live at home and in the community, by providing a planned program of activities directed at enhancing skills required for daily living. Centres also provide opportunities for support and social interaction as well as respite and support for carers.

CLINIC
A Registered Nurse provides Primary Nursing Care to residents, including assessment, advice and referral to appropriate agencies if necessary. Clinic operates at the Queenscliff Centre.

COMMUNITY DEVELOPMENT WORKER
Development of local support and self help networks and community projects.

COMMUNITY HEALTH NURSING
General counselling, health education, referral and treatment nursing service.

CONTINENCE ADVICE
Registered nurses provide assessment and advice on continence.

DENTAL
A general service providing general dental treatment to pensioner and healthcare cardholders.

DISTRICT NURSING
Provides nursing care to people in their own homes, including frail older people, mothers and infants, post acute care and palliative care enabling clients to maintain an optimum quality of life. Expert advice regarding management of diabetes, continence and wound care is available.

FAMILY COUNSELLING
A social worker provides counselling for families.

FAMILY SUPPORT UNIT
A unit attached to Coorabin Hostel. Provides respite for families caring for a family member with a terminal or chronic condition.

FOOTCARE NURSE
Provides footcare and feet & low leg massage.

HEALTH EDUCATION
Individual and group activities on health related topics.

HOME HEALTH AIDES
Aides provide personal care, respite and socialisation services to support people at home and delay the need to enter an institution.

MASSAGE
Private providers provide therapeutic massage.

MATERNAL & CHILD HEALTH NURSE
Maternal & Child Health Nurses are located at Ocean Grove, Queenscliff, Drysdale and Portarlington Centres.

MEALS ON WHEELS
Hot midday meal provided at home and in Adult Day Activity program with assistance from a roster of volunteers, in Borough of Queenscliffe area.

MEDICAL PRACTITIONER
(Fee for Service)
A general medical practice and a 7 day/24 hour per day emergency service at Queenscliff Community Health Centre.

NEEDLE & SYRINGE PROGRAM
Providing needle exchange & safe disposal units.

NUTRITION/DIETETICS
Accredited Practising Dietitians provide to both individuals and groups:
- Nutritional advice to maintain health
- Dietary advice and counselling to treat disease

OCCUPATIONAL THERAPY
People of all ages, with any health problem(s) are assisted to be as independent as possible in their daily activities. Advice and assistance is given in the areas of Adaptive equipment, alternate techniques and/or home modifications. Home visits, falls prevention, safety, rehabilitation therapy, stress management, support groups, splinting, access advice & equipment hire are provided.

PALLIATIVE CARE
Palliative Specialist nurses are available for individual family and group support and advice. The Palliative Care Service works in partnership with District Nurses.

PHYSIOTHERAPY
Individual and group treatment services are provided including post acute rehabilitation and maintenance therapy as well as Warm Water Exercises for people with arthritis.

PODIATRY
Deals with the prevention, diagnosis, treatment and rehabilitation of conditions of the feet and lower limbs.

PSYCHOLOGIST
Available for individual and family therapy.

RESIDENTIAL AGED CARE
100 Low level (Hostel) beds. (20 Dementia Specific and 16 Independent Living Units). Based at Coorabin, Point Lonsdale and Ann Nichol House, Portarlington. See over for details.

SOCIAL WORKER
Available for individual and family counselling.

SPEECH PATHOLOGY
Assessment and treatment of speech and language disorders in adults and pre-school children.

WELFARE - ADVICE
Counselling, support and referral for a range of personal and family problems.

WELL WOMEN'S CLINIC
Clinics are provided by Accredited Women's Health Nurses at Ocean Grove, Drysdale, Portarlington and St Leonards. The Women's Health Nurses can be consulted on a range of women's health issues such as pap tests, reproductive and sexual health, breast health, hormonal imbalances, menopause, health education, counselling and referral.
