Bellarine Community Health Inc relies
greatly on both voluntary and financial
assistance from members of the
community in order to maintain the range of services
& programs for which it is responsible.
Due to the number of requests received each year for
information regarding making a Bequest or Donation,
the above form has been prepared for your assistance
when making or altering your Will or in making a
Donation towards the work of the Service.
BCH Board wishes to formally record its sincere

support our Service.
