The Home and Community Care (HACC) Program provides in home and community residential services to support frail older people, people with disabilities and respite for their carers. The service aims to provide support that enhances the individual’s well being, improving their capacity to live independently in their home. The HACC programs are jointly funded by the Australian and Victorian governments under the HACC amending agreement of 1998.
Aims:
The aims of the HACC Program are:
- to provide a comprehensive, coordinated and integrated range of basic maintenance and support services for frail aged people, people with a disability and their carers; and
- to support these people to be more independent at home and in the community, thereby enhancing their quality of life.
The type of services funded through the HACC Program include, but are not limited to:
- nursing care
- allied health care
- meals and other food services
- domestic assistance
- personal care
- home modification and maintenance
- transport
- respite care
- counselling, support, information and advocacy
- assessment.
Target Groups
The HACC target group encompasses frail older people, people with disabilities of all ages, the financially disadvantaged and carers of these people. The HACC agencies program acknowledge the individuality of the person’s need when planning a collaborative support contract.
Specials needs groups are also recognised in broad HACC target population. There is a range of strategies to improve their access to services, inclusive of a responsive service design and well targeted information dissemination. The special needs groups are:
- people from culturally and linguistically diverse backgrounds
- people from Aboriginal and Torres strait islander backgrounds
- people with dementia
- people who are financially disadvantaged, and
- people living in remote and isolated areas.
HACC services can be provided specifically for one or more of the above groups either by a generic service or, where appropriate, by a specifically targeted service.
